How to Prepare for Your Appointment

Whether it's your first time coming to a podiatrist, or you've been a patient for years, we want to ensure that you feel comfortable and prepared! To get the most out of your appointment, we encourage all patients to plan in advance. Please be sure to print and complete all needed patients forms prior to your appointment. Then bring your completed forms, insurance card, photo ID, and a list of any medications or supplements with you.
